docs: list all packages under packages/ in CODEOWNERS - #372

Update .github/CODEOWNERS packages section to explicitly list all 10 packages under packages/ (app, cli, core, crypto, guard, hooks, policy, sandbox, shared, and tui) alphabetically with designated code owners, avoiding implicit fall-through to the wildcard rule.
