feat(context7): add managed OpenAPI adapter

• add a reusable managed OpenAPI adapter runtime with explicit operation allowlists and scope mapping
• integrate Context7 through dynamic OAuth client registration, PKCE, encrypted D1 credentials, and Adapter-issued DPoP tokens
• publish Context7 OpenAPI and Agent Skill discovery, plus deployment documentation
