Bounds a combined design set to one required valid independent pass and at most one second pass only for a defensible in-surface blocker. The optional pass uses unchanged bytes and a different lens before author fixes; there is no third or post-fix prose confirmation. Malformed-output retry remains separate and branch loop budgets are unchanged.
