Dotenv 17 logs to stdout

Dotenv 17 writes an injection message (and promotional tips) to stdout unless quiet is set. Both index and recommend preload dotenv/config without DOTENV_CONFIG_QUIET, so CLI runs will print extra output that was silent on v16. index.ts also calls config() again, so that command can log twice.
