Closes #457. Also tidies the Shortcuts menu, which is related to #458.

Toolbar (#457)

• Open, Project, Save, Browser, Focus, Preview, AI, Cite and Publication now show only their icon.
• Text is kept where it is needed: the HTML / PDF / DOCX export buttons, the "Export:" and "Font:" labels, A- / A+ and the font size box.
• Every icon button has a tooltip and an accessible label. Toggle buttons set aria-pressed.
• Tooltips show the action's current shortcut, e.g. "Save (Ctrl+S)". They follow the user's own key bindings and show Cmd on macOS. The old Save tooltip always said "Ctrl+S", even on a Mac.
• The Publication button now matches the other toggles. This also fixes its dark-mode classes: only the first class got the dark: prefix.
• Spacing is a bit tighter, so the toolbar takes less horizontal space.

Shortcuts menu

• The menu was a single list of 26 entries in mixed order. It now opens as a panel with File / Edit / Format / View columns.
• Related actions are kept together: headings in order, exports after save, Insert Table under Format.
• Columns wrap on narrow windows, and every entry is still clickable.
• Arrow keys now appear as Ctrl+Alt+← instead of Ctrl+Alt+ArrowLeft everywhere shortcuts are shown.
• The grouping logic is in lib/shortcut-groups.mjs and has Node tests, following the pattern of the other .mjs helpers.

Testing

• Checked in the web app: every toolbar button and toggle, light and dark mode, and clicking entries in the Shortcuts panel.
• tsc --noEmit, npm run lint, npm test (167 pass, 5 new) and npm run build all pass.
• Not tested in the Tauri desktop builds.
