feat(planning): fail a plan whose file:line citation does not exist, and stamp the commit it was checked at #10037

What to do
W5 of #6917. Add a script that reads a PLAN.md, resolves every path:line citation against a named commit, and fails on any path or line that does not exist; wire it into /planning:plan's outcome gate, and have plans record the commit the check ran at.
Done when
- The script's test exits 0 and includes a case where a plan citing a missing file, and one citing a line past the end of a file, each exit non-zero, plus a stay-quiet case for a valid citation.
