Targeted review of the new reconciliation/disposition records; this is not a claim that all 837 changed paths have been independently audited.
Please correct 010_reconciliation.md against the immutable trees: git ls-tree -d --name-only 57077ca3:devlog/_plan counts 157 and the head counts 57; _fin counts 429 -> 528. These are not the stated 158 -> 57 / 432 -> 533. Distinguish renamed units from already-present/merged duplicates so the terminal audit is reproducible.
More importantly, move #4121 out of CLEAR-FIX/no-PR into POLICY/upstream clarification in 030_issue_disposition.md. The issue proposes fabricating OpenCode client/session headers to pass an upstream gate whose message says that the free tier is OpenCode-only. Generic Zen interoperability documentation is not proof that this exact keyless tier authorizes that workaround. I left the supporting source/clarification request on #4121; the work queue must not pre-authorize implementing that bypass as a clear bug fix.
Date the open-issue inventory as an explicit capture time rather than a perpetual count; reconcile #4123 and #4126 if taking a new snapshot. Keep branch/worktree cleanup described as a historical record only—this PR must not authorize further deletions. No runtime code delta was found outside devlog, but docs-only CI does not validate these factual claims.

Closes #4149. The exhausted-combo 413 mapping no longer requires stream === true. It carries the loop's own classifyOverflow verdict forward through lastFailureClassifiesOverflow, so a hopping 413 (err_free_prompt_cap) is classified on both wire shapes while a local input_admission_refused keeps its own non-streaming diagnostic. A second commit repoints three deferred-verb route ownerDocs at devlog/_fin. The post-2.49 devlog reconciliation in #4125 moved those units and left the registry naming the old _plan paths, which turned "route exemptions stay honest" red on dev itself. Exact-head CI at 5095f1b: Cross-platform CI success, Enforce PR target branch success (run 34406302801, the latest at this SHA; the two earlier failures were the escaped-newline PR body, fixed before that run), PR hygiene success, PR Labeler success, React Doctor success. Only the CodeRabbit review bot is outstanding. The new regression asserts both combo targets were hit, which is what separates the exhausted path from the stop path already covered.
