This looks like a Chrome extension permission error rather than the captcha solver failing. Flow moved to flow.google.com in September, and the extension manifest still only requests access to labs.google, so it can't inject into the new host to mint the reCAPTCHA token — which surfaces as CAPTCHA_FAILED.

Two things that should help:

• PR fix(extension): make generation work on flow.google.com (captcha, tabs, batchexecute RPCs) #13 ("fix(extension): make captcha and token capture work on flow.google.com") appears to address exactly this, and is still open.
• Until it merges, adding https://flow.google.com/* to host_permissions in the extension manifest, and to the content script matches, then reloading the unpacked extension should restore token capture. Worth checking site access for the extension in chrome://extensions too. Step 5 of the README also still points at labs.google/fx/tools/flow.
