plugins/cache-tax/README.md: new section "The Mod form" between the what-it-does section and the native-behaviour section, linking cache-tax@claude-code-mods, stating the flag it needs, what it adds (refuse once by default,/keepwarm), that only one form should be installed, and that the status line segment stays with the hook's files.README.md: the cache-tax row of the plugin table gains one sentence linking the Mod form.site/index.html: card 14 gains one sentence with the same link.
