Upgrade Claude to Opus 5.5 and current Agent SDK

Move the Claude default from Opus 4.7 to the requested claude-opus-5-5, and upgrade the official Agent SDK from 0.1.77 to 0.3.287 (bundled Claude Code 2.0.77 → 2.1.287). Update runtime fallbacks, example configuration, README, and changelog consistently while preserving explicit user model overrides.

The adapter now recognizes the SDK's execution/turn/budget/structured-output error results and preserves their diagnostics. Successful task evidence records the runtime version and model IDs reported in actual assistant responses.

Validation on the Mac mini:

• Clean npm install; zero npm audit findings.
• Harness compilation, 59 unit/integration tests, example app typecheck, and production build passed.
• Real harness run with the Claude SDK and Opus 5.5 generated a Node-only sum library and tests, passed smoke checks, wrote independent evaluation evidence, and passed final regression. A final-code run also exercised generation/evaluation with response-model metadata enabled. No repair was needed.
• The live task evidence reports claudeCodeVersion: 2.1.287 and responseModels: ["claude-opus-5-5"] for both roles.

Reproduction: use a disposable workspace, provider claude-sdk, runtimeMode minimal, maxSprints 1, smoke.test node --test sum.test.mjs, and an evaluator MCP override of null for this non-UI test. Run node dist/cli.js run "Create sum.mjs exporting sum(a,b) and node:test coverage for positive, negative, zero and mixed-sign operands" --config <config.json> with existing supported authentication.
