Summary: re-enter triage_check when retrying at triage_gate; route errored approval-gate retries through regeneration; add regression tests. Previously the retry cleared the error but invoked the pause gate itself, which immediately paused again without rerunning triage. Validation: focused Ruff checks passed; all 43 worker tests passed.
