Add dot and dot-upper transforms - #129

-transform=dot produces my.type.value and -transform=dot-upper produces MY.TYPE.VALUE, using the same delimiter path as snake and kebab. dot_upper is accepted as an alias like the other underscore forms. Based on #61 by adenix.
