Anchor derived trace exclusions for FastAPI and FastMCP #158

Part of #156. FastAPI and FastMCP hand the default /metrics and the health-check path to OpenTelemetry's ExcludeList unanchored, so routes such as /api/metrics-report and /api/health/details produce no server span. Reproduction and suggested anchoring in #156.
