Numeric character references like ' and ' were left as literal text when parsing markdown, because decodeHtmlEntities only handled the four named entities it emits itself. Decode numeric and hex references too, and leave out-of-range code points untouched instead of throwing.
The chained replaces re-scanned their own output, so a decimal entity that produced '&' could be decoded again (e.g. &#x27; became '). Use one regex over a single pass so a replacement is never decoded twice.
Replace cellLineSeparator inside each rendered child before joining so a blank paragraph stays a blank line when the separator is two newlines. Co-authored-by: Bennett Payoyo <Yahiro025@users.noreply.github.com>
Invalid code points (zero, surrogates, above U+10FFFF) become U+FFFD, over-long references (more than 7 decimal or 6 hex digits) stay literal, and the C1 range (0x80-0x9F) is remapped to Windows-1252.
