Add configuration reference and explanatory section for two filtering mechanisms:
- [projects.*].exclude: file-level glob exclusion applied before audit
- [output.filter]: finding-level post-filter (rule_id/severity/category)
Documents the format ({mode}:{values}), supported wildcard patterns,
legal category values, and behavioral differences across CLI modes
(--baseline/--dir silently drop; --files/--manifest show as Ignored).
