Phase 1 — Working Forms (for the arpit-ae/axis repository)

This change is intentionally on a feature branch so the live goaxis.in site is not replaced until configuration has been checked.

Included

• Wire the homepage and service quotation forms to POST /api/enquiry.
• Add a Cloudflare Worker API route that validates fields, checks same-origin requests, requires consent, has a honeypot, and stores leads in D1.
• Add the Leads table migration and Wrangler configuration for Worker static assets + D1.
• Keep the existing static site as the asset fallback.
• Prevent deployment config, source and migration files from being served as public assets.
• Add GitHub Actions syntax and wiring checks.

Important before production deployment

• Verify the D1 database ID in the Cloudflare account that owns the goaxis Worker.
• Review D1 migration state before applying migrations/0001_phase1_enquiries.sql.
• Configure a Preview/staging deployment of this Worker before updating the production Worker or goaxis.in.
• Confirm the Worker deployment has the DB and ASSETS bindings, and test both forms plus database rows.
• The admin/customer dashboard, secure login, payment processing and email notifications are NOT included in Phase 1.
• Legal/privacy review and Cloudflare rate limiting/WAF should be completed before collecting real customers' information.
