fix: expose fallback model and refusals - #67

Release 0.7.10 with the SDK changes needed by ALPHA-2610. Expose the winning adapter on agent results for overview provenance, and classify Anthropic refusal metadata so explicit fallback discards partial refused output. Validated with deno task check: formatting, lint, 326 tests, and publish dry run. 68 live-provider tests were skipped without credentials. Assisted-by: gpt-6.1-sol Assisted-by: chatgpt-dot:lilac
