Issue

Closes #3691

Scope

• Regenerate an Ornn recommended skill only when exact loading returns a typed NotFound.
• Replace the stale request-local recommended ref and rebuild the request-local catalog.
• Return generated skill markdown immediately from the creator result, including when durable ref persistence is unavailable.
• Preserve AccessDenied, digest mismatch, transport, and other exact-load failures.

• The real local relay previously reproduced USE_SKILL_NOT_FOUND for the deleted Ornn ref. This change targets that exact failure boundary; no direct fake relay callback was used.
