feat(commands): consolidate contextual command discovery

Summary

Command discovery now uses one canonical catalog for grouped commands, contextual completion and palette navigation, help, onboarding, and the generated command reference. Expired paths return their replacement before any TUI or command side effects.

Context

• Closes Consolidate slash commands and add contextual discovery #178.
• Scope: command discovery and grouping in commands/core/TUI, with minimal CLI integration.
• The saved MCP/plugin runtime and goal-loop work remains separate. Provider activation, plugin loading, and MCP runtime behavior are unchanged.

Changes

• Compose built-in, configured, skill, and already-registered plugin commands with explicit collision precedence and aliases.
• Rank local context and recent commands; progressively browse groups and execute selected leaves with one Enter.
• Preserve arguments during completion, including edits inside grouped command paths; keep session resume as a picker until a selection is made.
• Generate docs from the catalog and retain detailed handler guidance for argument-rich commands. Check catalog/reference drift and reviewed screen baselines.

Validation

Rust commands use rustup run stable (Rust 1.99) and CARGO_TARGET_DIR=/tmp/coven-code-pr182-target, run from src-rust/.

• cargo build -p claurst; COVEN_BIN=/tmp/coven-code-pr182-target/debug/coven-code REQUIRE_TMUX=1 bash scripts/tui-tests/run.sh — 48 passed, including one-Enter palette execution and retired-path rejection.
• Independent review: command dispatch, completion, TUI integration, generated registry, and all changed screen fixtures; findings addressed.
• An initial post-link ACP smoke run timed out; direct startup completed in 0.08s and all four ACP smoke tests passed on recheck.
• Not run: authenticated provider calls; discovery is verified with local state and offline terminal checks.

PR Readiness

• Diff is limited to the intended files.
• Generated reference and golden fixtures were regenerated by their commands and reviewed.
• User-facing docs/help were updated.
• No secrets, credentials, unrelated local data, or logs are included.
• Remaining scope and verification limits are listed above.
