REQUEST_EXPIREDduring bootstrap. This draft adds only a regression reproducer: provisioning and bootstrap each remain within their own configured phase timeout, but their combined duration exceeds the original single request lease. The expected current behavior is red withREQUEST_EXPIRED; production code is intentionally unchanged in this first step so CI can prove the defect before the fix is added.
