Document idempotent-retry behavior added in prior-auth#142/#146: duplicate Claim.identifier (system+value) returns the existing ClaimResponse with no re-adjudication; identifiers missing system or value are non-dedupable; genuine changes require a new identifier with Claim.related[replaces].
