Reviews now retain accepted intent and changed expectations, PR conversations, stable finding identity and explicit re-review dispositions. Pinned source remains review data; discussion association does not automatically confer approval. Adds a generic repository check executor using a disposable container without network, host mounts or credentials.
Validation: 55 tests passed at this slice. A real L1 fixture executed two behavior tests with zero model calls and removed its container. Later stack tests cover all five re-review dispositions. This establishes collection/execution behavior, not model-review quality.
