Adding mixins throws Unknown provider

Hi, Can I get a little help on adding the mixins, I've tried the instructions at https://platanus.github.io/angular-restmod/PagedModel.html

That is:

donation = restmod.model('/donations', 'PagedModel');

Results in the following exception

Unknown provider: PagedModelProvider <- PagedModel <- raiselyDonation
